| Avoid

Please complete the form below. After
you click "Submit Registration," you will
have two options:
- Secure your registration using PayPal or a Credit
Card.
- Return a completed registration form in the alloted
time with a check.
After you electronically register, I will
e-mail you a registration form (Word 2000) along with
the event details such as times, directions, contact
information, hotel choices and personal items you may
need to bring.
With both payment options, please
complete the registration form and return it to the
address on the bottom. When I receive your completed
form in the mail, I will send you another e-mail confirming
your participation.
**IMPORTANT**
I usually process
all registrations within a 24 hour time period. If you
have registered for a clinic and have not received an
informational e-mail from me, CHECK YOUR "BULK"
FOLDER. If you still have not received anything from
me, please send an e-mail or call me at (760) 635-3994.

Refund Policy
If a participant cancels due
to any reason on or before 8 days of an event, a $25 administration
fee will be deducted from the refund. If a participant
cancels due to any reason before 7 days of an event a
$35 administration fee will be deducted from the refund.
If participant leaves early or fails to show, no refund
will be given.
